The Colorado Medicaid Program is a public health program for low-income families, including eligible adults and children ages birth to 19 years. Eligibility for any program is based on family size and household income. Children must be living in the home more than 50-percent of the time.

An application with wage and resource verification is required to determine eligibility. All Medicaid applicants must provide original proof of citizenship and identity.  If you do not qualify for Medicaid, we will process your application for CHP+.

Medicaid provides medical benefits including inpatient and outpatient hospital, physician services, well child checks, immunizations, vision care including prescription glasses, prescription medications, dental care and mental health care. For eligible adults over age 21, dental and vision benefits may be limited. Medicaid services can be back-dated 90 days for Medicaid-eligible applicants. Please note:

  • Adults may incur a co-pay for services. 
  • Medicaid recipients must obtain a primary doctor and see him or her for all medical needs that are not emergencies. 
  • Medicaid will not pay for emergency room service if the service could have been provided by your primary doctor.

Non-Citizen Emergency Coverage

Medicaid covers emergency-only medical treatment for non-U.S. citizens. Coverage is limited to the care and services necessary to treat an immediate emergency medical condition.  Eligibility for these benefits ends after the emergency service has been provided.

An emergency medical condition exhibits acute severe symptoms that without immediate medical attention could result in:

  • Placing the patient’s health in serious jeopardy;
  • Serious impairment of bodily function; or
  • Seriousdysfunction of any organ or body part.

Eligibility

To qualify for Family Medicaid benefits, you must meet these requirements:

  • Be a U.S. citizen or a legal permanent resident;
  • Be a resident of Colorado;
  • Provide your Social Security number;
  • Provide verification of earned income; and
  • If pregnant, have proof of pregnancy

If you are determined ineligible for Medicaid, your application will be processed for the Child Health Plan Plus program (CHP+).


How Do I Apply For Benefits?

There are several options available to you for obtaining and filing a Colorado Financial, Medical, and Food Assistance Application:

  1. Print the online application using the link above, or secure a copy at one of our  County office locations.
  2. Complete the application and fax, mail or deliver it in-person to either office location.
  3. If you are unable to go to the nearest office, you may have another person, (called an authorized representative) apply on your behalf. You must designate the authorized representative in writing.

What to Bring When Applying:

Bring as many documents as possible (must present original documents):

  • Proof of citizenship, such as:
    • U.S Passport, birth certificate, citizenship record, or Affidavit
    • Social Security card
  •  Proof of identification for each individual applying, such as:
    • Colorado drivers license
    • State ID or School ID
    • Affidavit for children under 16 years of age 
  • Proof of Colorado residency,such as:
    • Any photo identification with your address
    • School records
  • You may be requested to provide proof of your income.
